What bolt pattern/rims are those rotors for?

Look Closer, Those are 6 lug hubs as well.

My guess is an Isuzu NPR or Mitsubishi Fuso

That is what I thought also. I looked up the Japanese trucks, and they have kind of similar looking wheels also. How is the caliper going to be attached. It looks mocked up right now

Its definitely an M37, but it looks to me like he's adapting 6 lug hubs onto the stock spindle.

right on the money Brett. its a 6 bolt hub from a 57 d100 on a m37 spindle. I think the rotor and caliper are off a f550.
the nice thing is a flat plate on the spindle bolts will mount the caliper. the bad is its not a 2.5ton bolt pattern. he will need a wheel adapter and a minimum 18in wheel.
